What are the Kentucky Medicaid Waivers?

Kentucky, like many states, has several Medicaid-funded options for residents who have intellectual and developmental disabilities. In Kentucky, there are several major Medicaid-funded waiver programs available. Heartfelt Solutions is fully certified and credentialed to provide services within the Support for Community Living (SCL) and Michelle P Waiver (MPW) programs.

Person Center Perspective

At Heartfelt Solutions we approach waiver services from a person centered perspective. Families and their loved ones are involved in setting goals and determining what is important to and for their loved one.  Waiver services can help individuals flourish and improve independent living skills, social skills, money management skills and self advocacy skills. Skills are addressed within the home and community, creating realistic opportunities to learn and practice. Below you will find a list of individualized services that are available via the waiver and provided by Heartfelt Solutions

Michelle P. Waiver (MPW)

Michelle P. Waiver- The Michelle P. Waiver (MPW) was established as a way to expand access to services that are non-residential but similar to the SCL waiver. There is no age restriction with MPW so individuals can access services earlier in life. Our MPW Services Include:

  1. Positive Behavior Support: Services are provided by Master’s level clinicians who are often also licensed and board certified to provide ABA services. Services are evidence-based, relying on data and assessment results to develop a Positive Behavior Support Plan which identifies preventative strategies to reduce barriers and challenging behaviors. Addressing these challenges systematically while teaching replacement skills will help to improve activities of daily living, social interaction, and school/work life.
  2. Community Living Support (CLS): This service is designed to teach a variety of skills related to independent living, social/leisure skills, money management, cooking and other areas based on an individual’s personal goals and interests.
  3. Respite: Services are provided to caregivers allowing for a break from daily ongoing care.

Support for Community Living (SCL)

The Support for Community Living Waiver (SCL) is one of the original waivers designed to assist Kentucky residents with intellectual/developmental disabilities to stay in their local community and receive the housing and necessary service and support they need to be successful. This waiver is intended for persons 21 and older. Our SCL Services include:

Consultative Services (Positive Behavior Supports; Psychological)-The primary focus of these services are to provide expertise, training and technical assistance to improve the ability of caregivers to carry out therapeutic interventions and reduce challenges that may be interfering with individuals’ daily functioning and quality of life. Depending on the area of need, the service may be provided by the following: licensed behavior analyst, licensed professional clinical counselor, licensed psychological associate, licensed psychologist, licensed psychological practitioner, licensed clinical social worker, or positive behavior support specialist.

Person Centered Coaching: Services provided by a Person Centered Coach are designed to assist individuals in all environments when a barrier challenges further success or independence. The Person Centered Coach models and teaches person centered thinking for all members of an individual’s team and also works closely with the behavior specialist as it relates to data collection and analysis.

Personal Assistance: Personal Assistance is designed to assist an individual in accomplishing tasks that the individual would typically complete on their own if they did not have a barrier challenging the ability to complete those tasks independently.

Community Access: Community Access is designed to assist people with support, training and empowerment to participate in meaningful routines or events, hold a membership in a club, group, association, church, business, or organization in the community; and build a natural support system. Community Access emphasizes connecting with others, independent functioning, self-advocacy, and personal responsibility.
